چکیده

Objective: To evaluate and compare the protective effects of salbutamol and montelukast in amelioration of insulin induced airway hyper-reactivity on isolated tracheal smooth muscle of guinea pig in vitro. Study Design: Laboratory based randomized control trials (Experimental study). Place and Duration of Study: Pharmacology Department and Centre for Research in Experimental and Applied Medicine (CREAM) Army Medical College Rawalpindi from January 2012 to September 2012. Material and Methods: Effects of variable doses of insulin (10-7-10-3 M) and insulin pretreated with salbutamol (10-6 M) and montelukast (10-5 M) were observed on airways of guinea pig in vitro by constructing cumulative dose response curves. Four Channel Oscillograph and Transducer were used to record the tracheal smooth muscle contractions. Results: Insulin induced a dose dependent reversible contraction of isolated tracheal muscle of guinea pig. The maximum response of insulin and insulin pretreated with salbutamol and montelukast were 35 ± 1.13 mm, 14.55 ± 0.62 mm and 34.5 ± 1.024 mm respectively. So salbutamol significantly inhibited the contractile response of insulin while montelukast failed to counteract the airway hyper-reactivity induced by insulin. Conclusion: Salbutamol significantly inhibited the contractile response of insulin, while montelukast did not produce statistically significant effect on insulin induced airway hyper-reactivity. So it is suggested that pretreatment of inhaled insulin with salbutamol may be beneficial in inhibiting the broncho-constriction induced by insulin.

Montelukast attenuates the airway response to hypertonic saline in moderate-to-severe COPD.

This study assessed the effect of the leukotriene receptor antagonist montelukast on hypertonic saline-induced airway obstruction. A total of 29 patients with chronic obstructive pulmonary disease (forced expiratory volume in one second (FEV1), 42+/-4% predicted) received either 10 mg montelukast and 3 h later placebo via metered-dose inhaler (MDI) (M), or placebo and 3 h later 200 microg salbu...

Cysteinyl-leukotrienes in the regulation of β2-adrenoceptor function: an in vitro model of asthma

BACKGROUND The response to beta2-adrenoceptor agonists is reduced in asthmatic airways. This desensitization may be in part due to inflammatory mediators and may involve cysteinyl-leukotrienes (cysteinyl-LTs). Cysteinyl-LTs are pivotal inflammatory mediators that play important roles in the pathophysiology of asthma, allergic rhinitis, and other inflammatory conditions. We tested the hypothesis...

Leukotriene Antagonist Therapy a Randomized, Controlled Trial of Intravenous Montelukast in Acute Asthma

among asthmatic patients. Study Population. Thirty nonsmoking patients with mild/moderate asthma and baseline forced expiratory volume in 1 second values of 70% of predicted values. All patients had experienced no asthma attacks in the previous 2 months and controlled their symptoms with inhaled salbutamol only. Patients did not receive corticosteroids in the 6 months before the study and had n...

Dose reduction of inhaled corticosteroids under concomitant medication with montelukast in patients with asthma.

The present study aimed at comparing the effects of a dose reduction of inhaled corticosteroids on lung function, indirect measures of airway inflammation and clinical scores during treatment with a leucotriene receptor antagonist. In 50 patients (mean forced expiratory volume in one second (FEV1) 94% predicted), steroid doses (800 microg beclomethasone dipropionate) were first reduced to 50% a...

Failure of montelukast to reduce sputum eosinophilia in high-dose corticosteroid-dependent asthma.

Sputum eosinophilia is a sensitive predictor of benefit from corticosteroid treatment. Montelukast is a cysteinyl leukotriene antagonist, which also reduces sputum and blood eosinophils. The present study examined the possibility that montelukast has an added eosinophil-lowering effect in subjects with asthma who are corticosteroid responsive but relatively corticosteroid resistant. A total of ...
